We accept private pay and Title XX. You may be eligible for subsidized child care through the Department of Job and Family Services. Click here to learn more about the application process.

The following forms must be completed before you child can begin attending KidSpace:

The following forms need updated by all families annually. KidSpace management will advise you when updated forms are due.
• Medical Statement from Doctor
• Income Eligibility Form
• Child Enrollment Form (from ODJFS)
• Emergency Pick-up Form
• Medication Request Form (must be signed by doctor)
• Sunscreen Permission Form
